COLD CAP EFFECTIVENESS
Are cold caps effective with all types of chemotherapy?
Cold cap effectiveness during chemotherapy varies based on several factors such as the chemotherapy type, cold capping protocol adherence, and hair care practices. Individuals may metabolize chemotherapy drugs differently, as well as respond differently to stress and other emotional factors which can also influence hair loss. Although results cannot be guaranteed, rigorous protocol adherence can help to improve success.
Success rates range between 50% and 80%, depending on the study, indicating the amount of hair that was successfully maintained, however the overall Satisfaction rate is closer to 90%. Clients attribute this to factors such as maintaining privacy, promoting a sense of empowerment, and the much faster hair regrowth that is achieved after cold capping.
Without using cold caps, there’s a 100% chance of hair loss with most chemotherapy. Using cold caps allows some hair to be maintained, and because the follicles are protected, it offers accelerated regrowth as well. Contact us for questions specific to your treatment plan.

Is it normal to lose hair even with cold capping?
Hair loss typically begins between days 17 and 21 after the first chemotherapy infusion, even using cold caps. The cold cap protects the hair follicles but does not prevent hair loss completely. Older hair tends to fall out after the first treatment, which can be alarming but be assured this is normal. Hair loss amounts can vary due to factors such as chemotherapy drugs and dosages, an individuals drug metabolism, and hair growth phase cycles. Cold caps do not guarantee specific results, but without them the risk of alopecia is 100%. Protecting hair follicles is crucial for healthy regrowth and reducing the chances of permanent alopecia which can be caused by some Taxanes.
Should cold caps be used from the first treatment? What if I miss a treatment?
To maximize success, it’s important to start using cold caps from the first treatment to protect the hair follicles from chemotherapy damage. Some people who didn't use cold caps at the start of treatment may choose to start later to protect remaining follicles and promote accelerated, healthy regrowth.
Are cold caps effective with all types of chemotherapy?
The cold caps can be used for many types of chemotherapy for solid tumour cancers as well as lymphoma with oncologist approval.

COLD CAP USAGE
Are cold caps painful to wear?
The cold cap is very cold, but if the temperature is correct according to your hair volume and texture it should feel cool on the scalp but not painfully cold. Some clients find the first 5-8 minutes to be mildly uncomfortable until the scalp becomes numb, and then most tolerate it very well afterward.
If you have spine or neck conditions, please consult a healthcare professional before using the cold caps.
If you have spine or neck conditions, please consult a healthcare professional before using the cold caps.
How long are the cold caps worn for each treatment?
The cold caps should be worn starting 1 hour before treatment, throughout treatment, and then for 3-5 hours afterward depending on your drug regimen (specific timing provided individually).
Why do you recommend continuing to use the cold caps for 3-5 hours post-treatment?
Continuing to wear the cold caps after your treatment completes is crucial to give your body time to eliminate the chemotherapy to safer levels for your hair follicles. Penguin Cold Caps' portability means you can continue post-treatment in the comfort of your own home.
Are there any tips for better results?
- Be sure to make sure that it feels like the caps are making good contact with your scalp each time – no ‘baggy’ areas.
- Capping longer than we have advised can help to give your body more time to eliminate the chemotherapy drugs – even another 1-2 caps at the end of the day if you have the energy.
- Stay well hydrated especially the day before, day of and day after treatment to help flush out the chemo drugs
- Try to eat foods to help support your liver (general healthy things – google “liver detox foods”)
- Switch away from aluminum deodorant (some studies say the aluminum settles in the follicles which can weaken them)
- Consider biotin (10,000mcg/day) and silica supplements to help keep your hair as strong as possible. Please consult your doctor before taking any supplements.
- Try to reduce stress! This tip may be most important as stress can cause hair loss even in people who are not cold capping. Try not to let the capping process and/or shedding stress you – you will be through this before you know it, and there are lots of ways to camouflage any thinning if needed until the regrowth comes in (please see our Hair Care guide for tips). Understand that many clients experience some shedding 14-21 days after the first treatment and this is normal. Hang in there – typically the worst of the shedding is over in a week or so. Don’t give up – protecting your follicles with the caps means that each hair that sheds will grow back much faster when you’re done – you’ll be happy you stuck with it!
HAIR CARE
Can I cut and colour my hair before starting treatment?
Yes, you can cut your hair before starting treatment, although you are not required to do so. Getting a trim is recommended, to make it easier to take care of your hair and to help to minimize tangles. If you would like to colour before treatment it is recommended to use a gentle colour (no bleach or ammonia) and to do so at least one week before your first chemotherapy session to allow your scalp time to recover. This helps ensure the best possible cold capping results.
How often should I wash my hair?
Here are some guidelines for washing your hair while you're cold capping:
- Minimize Washing: It's recommended to wash your hair less than you normally would, ideally no more than once or twice a week. This helps reduce the stress on your hair follicles, which can become fragile during treatment.
- Gentle Products: Use a gentle, sulfate-free shampoo and a lightweight conditioner. Harsh chemicals and heavy products can weaken your hair and may contribute to hair loss. Note that baby shampoo is not recommended.
- Cool Water: Wash your hair with cool or lukewarm water. Hot water can be harsh and drying on your scalp and hair.
- Avoid Scrubbing: Be gentle when washing your hair. Avoid vigorous scrubbing or rubbing as this can pull on your hair and weaken the roots.
- Air Dry: After washing, allow your hair to air dry naturally. Avoid using a hairdryer, as the heat can damage your hair. If you need to speed up the drying process, use a cool setting on your hair dryer and avoid brushing your hair while it’s wet.
How can I style my hair? Can I use my hot styling tools?
- Avoid Heat Styling: It’s highly recommended to avoid using hot styling tools such as hot hair dryers, straighteners, and curling irons. Heat can weaken your hair and scalp, making it more prone to breakage and hair loss. If you must use a hair dryer, use the cool setting near your roots. Using a little heat on your ends is ok.
- Gentle Styling: Opt for simple, gentle hairstyles that don’t pull or stress your hair. Loose ponytails, buns, or braids are good options. Avoid tight hairstyles that could tug on the hair follicles, like tight buns or ponytails.
- No Harsh Treatments: Avoid chemical treatments, such as coloring, perming, or relaxing your hair during your treatment. These processes can severely damage your hair and scalp, counteracting the benefits of cold capping.
- Use Soft Hair Accessories: Choose hair accessories that are soft and don’t cause tension on your hair, like fabric-covered hair ties, wide-tooth combs, or soft hairbands. Metal clips and elastic bands can pull on your hair and cause breakage.
Can I swim in a pool or lake during treatment?
Swimming during chemotherapy while cold capping requires careful consideration to protect your hair and scalp. Here’s what you should keep in mind:
- Chlorinated Pools: Swimming in chlorinated pools is generally not recommended during cold capping. Chlorine can be harsh on your hair and scalp, potentially causing dryness and irritation. This could weaken your hair, making it more susceptible to shedding, which might reduce the effectiveness of cold capping.
- Natural Bodies of Water (Lakes, Oceans): While natural bodies of water like lakes or oceans may seem less harsh, they still pose a risk. Lake water may contain bacteria and other impurities that could irritate your scalp. Saltwater from the ocean can also be drying and harsh on your hair, similar to chlorine.
- Precautions if You Choose to Swim:
- Cap Your Hair: If you do decide to swim, wear a well-fitting swim cap to protect your hair as much as possible from water exposure.
- Rinse Immediately: After swimming, rinse your hair thoroughly with cool water to remove any chlorine, salt, or other residues. Follow up with a gentle, sulfate-free shampoo and conditioner to help restore moisture.
- Limit Exposure: Try to limit your time in the water, and avoid frequent swimming during the course of your treatment.
Can I exercise during treatment?
Yes, you can absolutely exercise during chemotherapy cold capping, and staying active can be highly beneficial for your overall well-being. Here are some tips to help you exercise safely and effectively while protecting your hair:
- Embrace Gentle and Moderate Activities: Low-impact exercises like walking, yoga, cycling, or swimming can be great choices during treatment. These activities help maintain your strength, flexibility, and energy levels without putting undue stress on your body or hair.
- Protect Your Scalp: When exercising outdoors, be sure to protect your scalp from sun exposure by wearing a loose, breathable hat or staying in the shade.
- Hydrate Well: Staying hydrated is key during exercise, especially when undergoing chemotherapy. Drink plenty of water before, during, and after your workout to keep your skin hydrated as well as support your body’s recovery and maintain your overall health.
Can I use hair ties or clips or wear hats, helmets, or other head coverings?
Yes, you can use hair ties, clips, and wear head coverings while cold capping, but it’s important to do so with extra care to protect your hair and scalp. Here are some guidelines to help you:
- Hair Ties and Clips:
- Gentle Accessories: Use soft, fabric-covered hair ties and avoid tight elastics or metal clips that can pull on your hair and cause breakage. Opt for loose styles like a low ponytail, loose braid, or simply letting your hair down.
- Avoid Tension: Be mindful of tension on your hair. Tight hairstyles can put stress on hair follicles, increasing the risk of hair loss. Loose and gentle is the way to go.
- Hats and Head Coverings:
- Soft and Loose Hats: If you want to wear a hat, choose one that is soft and loose-fitting to avoid putting pressure on your scalp or causing friction. Cotton or silk-lined hats are good options because they are gentle on your hair.
- Helmets: If you need to wear a helmet (e.g., for cycling or sports), try to minimize the time it's worn and ensure it fits comfortably without pressing too tightly on your scalp. Consider using a soft liner underneath the helmet to reduce friction.
- Protecting Your Hair:
- Limit Wearing Time: Try to limit the time you wear any head coverings to avoid excessive rubbing or pressure on your hair and scalp.
- Avoid Wet Hair: Make sure your hair is dry before putting on any head covering to prevent the scalp from becoming too warm or moist, which could potentially weaken hair.
DRY ICE
Why do I need dry ice? Can I use regular ice instead?
Dry ice is necessary to maintain the extremely low temperatures required to cool the cold caps in order to be effective. Regular ice is not cold enough and cannot maintain the consistent temperature needed, which is essential for optimal results. Please use our Dry Ice Locator to find a supplier near you.
How much dry ice do I need? Where can I find it?
We recommend obtaining 60 lbs of dry ice in pellet form the day before your treatment. This will ensure that you will have 50 lbs remaining at the start of your treatment day, as some dry ice will evaporate/sublimate overnight. Please use our Dry Ice Locator to find a supplier near you.
How do I store dry ice?
Store the dry ice in the cooler with the lid closed, using a towel to take up any air space inside the cooler. This method helps to slow down the sublimation process and maintain the dry ice for as long as possible. Keep the cooler in a well-ventilated area, away from sleeping areas as well as children and pets, to ensure safety.
IMPORTANT: Store the dry ice in the cooler only, do not store in your fridge or freezer as the extremely cold temperature could cause the thermostat to malfunction, and the air movement inside the fridge/freezer will also cause the dry ice to evaporate/sublimate faster.

BILLING
How does the monthly billing work?
You are billed 30 days in advance with our auto-subscription model. You only need to place an order for one month, and it will automatically renew each month, charged to the method of payment used for the order. When you return the kit, the billing cycle ends, and you will receive a prorated refund for any unused days beyond the 30-day minimum rental.
Why did I get charged again after I shipped my kit back?
Returns are typically processed within 2-3 days of delivery and are based on the delivery date, so if you are billed for the next month rental you will receive a refund for the unused days in your billing cycle.
TROUBLESHOOTING
Why is my thermometer is reading "LOW"?
The infrared laser thermometer will read "LOW" if the temperature reading is below -40 C. Set the cap outside the cooler, or massage the cold cap with your hands and/or forearms to warm it before taking the temperature again.
Why is the infrared thermometer is reading different temperatures from different areas of the cold cap?
There are a few things that can affect accurate readings. Here are a few things to know:
Moving the thermometer around on the cap and taking the temp of different areas even a few cm away often does tend to produce slightly different readings – it’s tough to get it exact and it’s not actually crucial to either. Being a few degrees on either side of the temp you’re aiming for is ok, aim for the colder end of the range if you can or even a few degrees colder – this is fine as long as it feels tolerable to you.
If you measure the temperature on the inside if your arm (hold it about 6” away) it should read about 30C – that’s a good way to gauge if the thermometer is working correctly.
If you feel there is an issue with the IR thermometer in your kit please let us know. Typically most issues can be corrected with fresh AAA batteries, however the manual thermometer is there in case of emergency. To use the manual thermometer, simply open the thermometer and lay the probe in the center of the cap. Fold the cap in half over the probe, being careful not to puncture the cap. The temperature should be displayed within 10s. Email us at [email protected] to inform us of the issue and a new one will be sent to you right away.
- IR thermometers take only the surface temperature of the object you are aiming at, not internal temperature (be sure to remove the caps outer bag)
- Frost can affect the reading (be sure to give the cap a quick wipe)
- Make sure there isn’t anything on the lens of the thermometer (if so wipe with an alcohol wipe)
- The area that is being measured increases the further away you are from the cap – be sure to be only about 6-8 inches away
- Hold the thermometer at a 90 degree angle to the cap
- Hold the thermometer still
- Pull and release the trigger quickly
- Make sure the thermometer is in Celsius/C mode
- Be sure not to change the settings using the buttons on the front (other than the C/F button and the “triangle” button that turns the red dot on/off)

The cold caps are coming out of the cooler too cold - how can I warm them to the correct temperature quickly?
Take the caps out of the cooler a few minutes before you expect to use them to use the ambient room temperature to help warm them up. In the event that you need to warm them more quickly, use your hands or forearms to knead/massage them. Wiping them off with a towel will help to remove any ice crystals which will aid in the warming process.
